Physical Examination: Certification by a physician that the student is able to participate without restriction in the sports named above is required each year. The physician’s signature below certifies that the student named above is able to fully participate in sports.

Insurance Requirement: Under state law, school districts are required to ensure that all members of school athletic teams have accidental injury insurance that covers medical and hospital expenses. This insurance requirement can be met by the school district offering insurance or other health benefits that cover medical and hospital expenses. Some pupils may qualify to enroll in no-cost or low-cost local, state, or federally sponsored health insurance programs.  Information about these programs may be obtained by calling 1-800-747-1222 (toll free number).

The Athletic Program Enrollment Form MUST be signed and dated by a physician each year and turned into your site's Athletics Office.
 
All students must have a completed copy of this form on file at their school site prior to participating in any school sponsored athletic activity.  This requirement includes all student managers, assistants and scorekeepers/ statisticians.  No student may practice or engage in any school sponsored athletic activity until this form is returned to your school site.  All sections of the form must be completed.  In some cases information will be requested more than once on the form as part of this form is given to each coach during the season.
 
The Governing Board expects district employees to maintain the highest ethical standards, follow district policies and regulations, and abide by state and federal laws. Employee conduct is expected to enhance the integrity of the district and advance the goals of the educational programs. Each employee is expected to make a commitment to acquire the knowledge and skills necessary to fulfill his/her responsibilities.

To be a chaperone, please download, sign, and return this form to your site administrator.
Your child may be eligible for no-cost or low-cost medical, dental, and vision care coverage, even if you work, are self- employed, own your own home, or are married. There are even programs for children and adults without legal status.

If you are interested in the Healthy Kids Project, please download, fill out this form and return it to the address indicated on the form. 
 
Students are frequently photographed when participating in school activities, and these photographs may appear in such District publications as school newsletters, the School Accountability Report Cards, or the school/district websites. These publications are frequently distributed outside the District. For example, copies of School Accountability Report Cards are available at the City Library and are distributed to all parents in the community. Students may also be videotaped when participating in school activities, and occasionally these videotapes may be distributed in some manner to members of the general public, such as being shown on Cable TV.

In order to ensure every student’s right to privacy, we are asking you to fill out and return this form if you do not want the District to publish and/or distribute any photograph or videotape in which your student appears. (This does not apply to yearbook photos or school newspapers published on the Internet.)

If you DO NOT want the District to publish and/or distribute any photographs or videotape in which your student appears, please download this form, sign it, and return it to your site administration.  
 
This permission slip allows for your student to be transported to the school-sponsored event(s) listed on the form by a parent, district employee, or other adult who is on the District’s approved driver list. (You must sign the Hold Harmless statement.)

Please read this document carefully. When signed by you, and if appropriate, your guardian/parent, it becomes a legally binding contract. You will be required to return this signed contract to the school before you will be given access to our computers and network. If you have any questions concerning this agreement, please call your principal’s office.
 
Volunteer Driver Application Instructions  (Please download the form to your computer if you wish to sign it electronically)
Only those parents, school district employees, or other adults who have been approved by the district may transport students to and from school activities in private or district-owned vehicles. Applicant agrees to transport students to school-sponsored events only after the district receives all required documentation and approves the application. District policy does NOT permit students to drive other students to events. School district insurance does not provide bodily injury or property damage liability coverage for use of private vehicles.

Documents Required:


Attach a clear copy of your current (within the last 30 days) CA DMV Driver Record Report (DMV points acquired within the last 3 years automatically disqualify you to drive).

Attach a clear copy of your California driver’s license.

Attach a clear copy of the page of your auto insurance policy that shows your name, the policy number, and verification of current coverage amounts (minimum requirement is $100,000 / $300,000 in bodily injury liability).

Complete this form and sign the Hold Harmless statement.

District policy permits students to transport themselves and siblings to and from school sponsored activities under special circumstances with parent permission (BP 3541.1). Completion of the information below will authorize the student named below to act accordingly.
