1. Click on the link labeled “Athletic Program Enrollment Form”. (Please note, you will need Adobe Reader to open this file)
2. Once you have opened the “MVLA Program Enrollment Form” Type-In the appropriate information designated by each line. (It helps if you have enabled “Highlight Fields” on the top right-hand tab of your Adobe Reader)
3. Once all 4 pages have been filled out, please double check that the information you have provided is correct and completely filled out.
4. Please print two copies of each of the 4 pages.
5. Make sure both the Student-Athlete and Guardian signs all appropriate signature & initial lines.
6. Please schedule an appointment with you local physician and/or attend the School’s Hosted Pre-Participation Physicals Exams on Saturday August 8th, 2009 at Restor Physical Therapy Clinic in Mountain View. (Information attached)
* The Pre-Participation Physicals that Mountain View High School will be hosting is not a required event. If a student-athlete or his/her guardian would like to go to their family physician to administer the pre-participation physical exam, that would be acceptable. However, one nice thing about the Mountain View Sports Medicine Staff proctoring the physicals is that the staff members can establish a baseline of each student-athlete in his/her specific sport; In the future, if something comes up or happens, we can look back at our records and decide the best possible treatment of an injury if it occurs. If you see you family physician, please have them check "high prone injury" areas regarding their specific sport. (I.e. Running: hip, ankles, feet, calves, ect; WaterPolo: shoulders, knees, ect; Football: knees, shoulders, ankles, hips, ect; Volleyball: shoulders, knees, backs, ankles, ect.) This way we can get a better understanding on minor areas that might turn up to be problems in the future.
**In addition, the pre-participation physical exam is only good for one year, and needs to be administered every year do to state bylaws. In order for the pre-participation physical to be valid, (By the State of CA) the exam needs to be conducted two weeks after the last, high school spring sports season league game. So if you do plan on seeing your family physician, please schedule the physical exam appointment after June 14th, 2009.
7. Please have a physician sign page two of the “MVLA Program Enrollment Form” to certify that the student-athlete has undergone a physical examination after June 14th, 2009 and is able to fully participate in high school athletics.
8. Once completed, make two copies of the form; Give one copy to your coach on the first day of try-outs and keep the other copy for your records.

Please make sure you bring a completed “MVLA Program Enrollment Form” signed by the student-athlete, guardian, and physician, to your first day of try-outs; Failure to do so would result in getting cut from that team.
