Attention ALL Los Altos High School Athletes
2009-2010 SPORTS PHYSICALS
This sports physical is good for the entire 2009-2010 school year. All fall, winter, and spring athletes should get their physicals now for participation in next school year’s sports.

- Where: Mountain View Physical Therapy Clinic

- Time: 10:00 A.M. – 12:45 P.M.
Sports Physicals are $35.00 each*.
This sports physical examination includes blood pressure, height & weight measurements, vision screening, and a general examination.
Class of 2013 : 10:00 a.m. – 10:45 a.m.
(A-M: 10:00 – 10:25 & N-Z: 10:25-10:45)
Class of 2012 : 10:45 a.m. – 11:15 a.m.
Class of 2011 : 11:15 a.m. – 11:45 a.m.
Class of 2010 : 11:45 a.m. – 12:15 p.m.
Each player must complete all of the following before he/she may participate in any sport:
- Complete the four (4) page Athletic Program Enrollment Form (enclosed)
- Pay the participation fee of $100 per sport directly to the LAHS bookkeeper and obtain receipt. Payments by check must be made payable to Los Altos High School Athletics. Print the student’s name and sport on the check.
- Purchase your ASB (Associate Student Body) sticker/card in the fall 2009
Bring the completed form with $35 check payment for the physical with you at the time of the examination.
If you miss the May physicals, there will be another one scheduled on August 13, 2009 from 5:30-7:30 pm at the Spinal & Sports Care Center on 2290 El Camino Real, Mountain View (650) 967-1152
