ATHLETIC PARTICIPATION REQUIREMENTS
The following information must be read and the attached forms must be completed legibly for eligibility to participate in the Los Altos Athletic Program. Please use either black or blue ink pen - no pencils or other colored ink as it is difficult to read and make photocopies of the forms.
I. Los Altos High School Athletic Participation Form.
1. Participation/Transportation - The participation fee ($100) must be paid for each sport. Make check payable to Los Altos High School and submit payment to the Bookkeeper. Please print your student’s name, student number, and sport on the check. NOTE: If you change your sport, it is your responsibility to contact the bookkeeper to make sure that your payment is correctly coded for that sport. If you would like to pay the participation fee at the time of the physical examination, please staple the check to the blue card. Cash payment will not be accepted. Fee waiver forms are available in the Administration Office and the Athletic Office. Complete and return this form to the Bookkeeper for processing.
Purchase an ASB (Associated Student Body) Card – proceeds pay for athletic league fees, officials, etc.
2. Physical Examination - form MUST be signed and dated by a physician each year (June 1 or after).
3. Insurance Requirement - A parent/guardian must complete this section and sign/date it. If the student is covered by Medi-Cal, the number used to apply for Medi-Cal must be stated. If the student does not have coverage, insurance may be purchased through an independent plan. Applications are available in the bookkeeper’s office and in the Athletic Office.
4. Acknowledgement - Parent/Guardian must sign giving consent for student participation.
5. Co-Curricular Pledge and Drug, Alcohol, & Steroid Policy - Parent/guardian and student must read both the Pledge and Co-curricular Drug and Alcohol Policy and by signing acknowledge that by breaking any portion of this agreement may result in negative consequences (i.e. dismissal from a team).
6. Eligibility Rules - Students must fulfill grade eligibility. Students/Athletes must have a 2.0 GPA, have passed 20 credits and be enrolled in 5 classes in the quarter prior to the season. Coaches will be informed of any student who has not received at least 2.0 GPA during the last grading period.
Football Age Rule only applies to athletes who are trying out for varsity football.
7. Emergency Information must be filled out completely and signed by parent or guardian. A copy of this will be given to each coach so that it will be available at all practices and games in case of an emergency.
II. Health History / Athletic Pre-Participation Screening Exam
1. Part 1 must be completed by parent/guardian prior to student’s physical examination.
2. Part 2 is to be completed by examining physician.
ALL FORMS are to be turned in to the Athletic Office. Please make sure you have completed ALL sections. If not completely filled out, your application to participate in a sport will be denied.
